Perceived learning needs of patients with heart failure in China : A cross-sectional questionnaire survey

Abstract
Aim: To identify the learning needs of patients with heart failure (HF) in China and to examine the relationships between learning needs and patients' characteristics.
Background: Little information on the learning needs of patients with HF in China is known. Besides, it is important to identify the potential characteristics that may influence patients' perceived learning needs.
Methods: This cross-sectional questionnaire survey was conducted in three university affiliated hospitals in Xi'an, Mainland China. A total of 347 HF patients selected by convenience sampling were investigated using the Chinese version of the Heart Failure Learning Needs Inventory. Descriptive statistics, independent t-test, Pearson correlation, and one-way analysis of variance were used for data analysis.
Results: The mean scores rated by Chinese patients with HF were lower than the corresponding scores rated by patients of Western countries. Information on risk factors, prognosis, general HF information and medications were identified as the most important topics while lifestyle modification and psychological factors were regarded as the least important ones. Age negatively correlated with the learning needs on activity (r=-0.125) and prognosis information (r=-0.141). Educational level was significantly related with the learning needs on risk factors. No other characteristics consistently predicted learning needs.
Conclusion: Patients with HF in China considered most of the topics important for them to learn. Age and educational level were related with the learning needs on only three topics of activity, prognosis information and risk factors. The incorporation of the study findings is critical to efficiently achieve the education goals in the limited hospitalization period.
Relevance to clinical practice: Differences of the study findings could be attributed to the special culture and healthcare practices in China. Health professionals can be informed by the study findings to develop the tailored educational programme for patients with HF in China.

Heart failure (HF) is a major and increasing public health problem in most parts of the world (Hodges 2009) including China (Gu et al. 2003). It imposes heavy physical, psychological and financial burdens upon patients (Lee et al. 2004, Friedmann et al. 2006). Numerous studies have shown that disease management programmes were effective in lightening the burdens of people with HF (Naylor et al. 2004, Yu et al. 2006). Moreover, health education has been regarded as the essential component of these management programmes to achieve positive results such as decreased readmission rate and improved self-care ability (Berkowitz et al. 2005, Shearer et al. 2007). However, time for HF education is diminishing as the length of hospitalization decreases due to the advanced management strategies (Chen et al. 2002). To improve the effectiveness and efficiency of education in the limited time, targeting patients' learning needs becomes crucial especially in China (Chan 2006). However, little information regarding the learning needs of Chinese patients with HF can be identified from the available literature. Hence, it is important to understand the perceived learning needs of patients with HF in China.
